Sandwich Man Killed in Forklift Accident at Sycamore Menards

Sycamore, IL — A forklift accident killed a Sandwich man Tuesday afternoon, March 27, 2012, at the Sycamore Menards around 2:30 p.m., reported the Northwest Herald.

Evaristo Alvarado, 56, of Sandwich, was operating a forklift when he became trapped in the machine and was subsequently crushed.

Responding emergency crews from the Sycamore fire and police departments rushed to the scene, where they found Alvarado, who had turned blue and was not conscious or breathing.

The DeKalb County Coroner pronounced Alvarado dead at the scene in the pack storage lost of Menards.

Details surrounding the workplace accident are still being released.

Alvarado, who is a Menards employee, did not actually work at the location where the deadly workplace accident occurred. Alvarado worked at the Menards distribution center in Plano and was at Sycamore location to do maintenance work.

The U.S. Occupational Safety and Health Administration (OSHA) are investigating the fatal accident.
