Orlando, FL — A woman was killed at the Amway Arena on Tuesday morning, May 3, 2011, when she was struck by a metal scaffold that fell from a forklift. The fatal workplace accident occurred around 10:15 a.m., near the west loading dock, reported the Orlando Sentinel.

According to police officials, Tracy Irwin, 58, was hit by the scaffolding although someone else at the scene tried to warn her about the falling construction equipment. She was apparently unable to move out of the way fast enough and was subsequently hit and seriously inured.

Responding emergency crews rushed Irwin to Orlando Regional Medical Center, where she died just after midnight.

Irwin was reportedly working for a subcontractor for Cirque du Soleil, which has been renting the arena.

Investigations into the workplace accident are underway. Investigators with the U.S. Occupational Safety and Health Administration (OSHA) are expected to arrive at the scene and conduct a full investigation.
